BACS Authorisation Form (click here to download)
This is required to authorise Howard James Recruitment Limited to pay you into a nominated bank account. We regret that Howard James Recruitment Limited does not pay by cheque payment.
National Insurance number
Must be provided on the BACS form as proof of your right to work in the UK, failure to provide NI number will hold payment.
P38, P45 or P46 Tax form.
P45 – this will be provided by your current / last employer
P46 – complete if you do not have a P45 (click here to download)
P38 – complete if you are a full time student (click here to download)
Copy of Driving Licence or Passport showing your photo
This is a proof of identification, I am sure you understand the need in this day and age.
Applicant Declaration Form (click here to download)
Proof that you have no criminal record and confirmation that Howard James Recruitment Limited introduced you to the client.
Contract for Services (click here to download)
This is the agreement between Howard James Recruitment Limited and the Temporary Worker.

All employment is subject to the receipt of satisfactory references, please provide details of your last two employers
Howard James Recruitment Limited reserves the right to withhold payment of wages until the above noted documents are completed in full, signed by you the temporary and returned to our offices for processing.
